Dr. Robert Canida, D.D.S.

2nd Generation Dental Pro

Bob was introduced to dentistry at a very early age through his father’s dental practice in Madison, and now has been joined in practice by his son, Ben. He is a 1967 graduate of Madison High School. He is a graduate of Taylor University and the Indiana University School of Dentistry. Bob completed the L.D. Pankey Institute for Post Graduate Dental Education, and also a year long Mini Residency in Oral Facial Pain under Dr. Jeff Okeson at the University of Kentucky College of Dentistry. He also actively pursues regular continuing education in many areas.

 

As a member of Calvary Baptist Church in Madison, Bob has participated in several missionary trips to foreign locations including Sierra Leone, West Africa, Honduras, Panama, Guatemala, Nicaragua, Haiti, and the Amazon in Brazil. Bob enjoys athletic activity. He has completed several bicycle trips across America as a dedicated participant with the Wandering Wheels, where he met his wife Charlotte. Besides cycling, Bob also loves water skiing, rowing, racquetball and volleyball.

 

Aside from his dental and athletic endeavors, Bob places great importance on community involvement. He is a member of the Human Relations Commission, a board member of Historic Madison, Salvation Army, and La Casa Amiga. He enjoys planting and watching trees mature. He is proud to be part of a group of 12 dentists and 9 hygienists who have started a free community dental clinic at the Salvation Army to serve those in need in the Madison area.

Dr. Benjamin Canida, D.D.S.

3rd Generation Dental Pro

Dr. Ben Canida grew up in Madison and attended Eggleston Elementary School and Madison Consolidated High School. He went on to earn his bachelor’s

degree from Taylor University and his professional degree from the Indiana University School of Dentistry. Dr. Ben has also received postgraduate training at

the L.D. Pankey Institute and the Dawson Academy, both recognized worldwide for their excellence in advanced technical reconstructive dentistry focusing

on severe wear problems, TMJ and cosmetic dentistry.

 

Dr. Ben joined his father in practice in 2005 carrying on the tradition of his grandfather as he became the third generation in the Canida family to serve the community of Madison

in the practice of dentistry. As a member of the dental community Dr. Ben is a member of the American Dental Association, the Indiana Dental Association, the Academy of General Dentistry, and represents this region as a delegate to the Indiana State Dental Convention. Dr. Ben also serves on the board of directors for the Community Foundation and the Heritage

Trail Conservancy.

 

Dr. Ben and his lovely wife, Becky, have three beautiful young daughters, Lilly, Ava and Kate. Becky is currently molding young lives as a preschool teacher at Madison Preschool.

In his time away from the office, Dr. Canida especially enjoys, crossfit, running, biking and waterskiing. He and his family are actively involved with Calvary Baptist Church.

Vicki Black

Front Office Manager/Insurance Coordinator

Vicki is a lifelong resident of Madison.

She attended Ball State University in Muncie, Indiana and Indiana University Southeast in New Albany, Indiana. Vicki brings a great deal of patience and love to her work here at Canida Dentistry. She is responsible for handling patient insurance claims, appointment scheduling

and payment services.

 

When she is away from the office Vicki is having the time of her life with her kids and grandchildren, volunteering at the Boys & Girls Club, baking bread, and passionately supporting Huntingtons Disease research. Vicki is a member of North Madison Christian Church.

Christy Facemire

Clinical Office Manager/Assistant

Christy is a lifelong resident of Madison and has been with Canida Dentistry since 1987, having worked at every position in the

office throughout her tenure. She has a degree in Medical Assisting from Ivy Tech and a certificate in radiology. She lives in

the Rykers Ridge community of Madison with her husband Steve and their two children, Amelia and Branson, who is a cancer survivor.

 

When she is not helping to better the teeth of Madison’s residents, she enjoys horseback riding, dark chocolate and helping with her children’s PTO school organizations. Christy also finds time to wrangle eight cats, three horses, three dogs and one pig!

Jackie Fouts

Hygienist

Jackie is a resident of New Washington, Indiana., where she lives with her husband Steve. Together, they have four grown children, Dave, Nathan, Sam and Liz, and three grandchildren. Jackie holds a degree in dental hygiene from the University of Louisville and is a graduate of New Albany High School. Back in 1976, Jackie began working for Bob Canida at Canida Dentistry and has provided amazing customer care throughout the growth of the practice.

 

Aside from her dental work and running Berry Farm with her husband, Jackie enjoys gardening, her grandchildren and her dog (not necessarily in that order).

Natasha Melton

Hygienist

Natasha is a licensed dental hygienist in

the State of Indiana. She graduated from the University of Southern Indiana in 2007 and within the same year began practicing her profession. Natasha's experience, attitude, and knowledge of her chosen

field simply reflects the quality of care she can provide.

 

When away from the office Natasha enjoys the outdoors, running, doing crossfit, and most of all spending time with her husband Seth and daughters Sara and Nora.

Office Policies

Patients are seen by appointment only. Your scheduled appointment time has been reserved specifically for you. We request 24 hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 24 hours in advance, you may be charged an additional fee.

 

If you are a new patient, you can save time on your trip to the dentist by downloading and filling out these forms below before your first dentist appointment.

Despite careful scheduling, emergencies can cause delays. We try our very best to stay on time. If your appointment time is affected due to an unforeseen emergency, we’ll try to notify you. We know that your time is valuable, too. You will receive the same quality care no matter how our schedule is running.

 

Fees & Payment Policies

Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. We accept Visa, Mastercard, Personal Checks and currency for your payments. Please ask a staff member and we will review our payment options to accommodate your financial needs. If special arrangements are needed, please talk to our office manager prior to receiving service.

 

Dental Insurance Policies

We believe that important decisions about your dental care should be made by you and your dentist, not your insurance company. Once you have decided on the course of treatment that you want, we will submit your claim and help you maximize your insurance benefits. Because insurance benefits vary greatly, we can only estimate your coverage in good faith, but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid

at the time of service. As a service to our patients, we will bill insurance companies for services and allow them 45 days to render payment. After 60 days, you are responsible for the entire balance, paid in full. If you have any questions, our courteous staff is always available to answer them.
